REGISTER FOR TRAINING:

• Choose course and method (online blended learning or in-person).
• Email the required official two page application (see forms) to RN Trainer Wendy@MyHolisticLifestyle.com.
• For renewals email the completed skills checklist (by certified, experienced Supervisor, LPN or RN)
• C2 Renewal and C3 Renewal skills to be done by RN Trainer in person or the Delegating Nurse in person - THE NURSE TEACHING THE COURSE IS NOT THE DELEGATING NURSE).
• When ALL paperwork is fully completed and recieved, look for your emailed copy of the certification issued by the Ohio DODD.
• Confirm on OH DODD Website.
